Reporting an Incident or Injury

Washington Hospital Services now allows our member hospital and client employees to report workplace incidents and injuries online.

By clicking on the link below, REPORT AN INCIDENT OR INJURY, you will be taken to a welcome page where you can begin the process of submitting a new incident report. Completing the online form will create an Incident/Accident Report that will be sent to your employers designated contact and your supervisor.

If you were injured as a result of the incident and sought medical treatment or plan to seek medical treatment, be sure to select “Yes” when the form asks you if you have sought, or intend to seek, medical care. Answering “Yes” to that question will initiate the completion of a Self-Insurer Accident Report (SIF-2), a claim for workers’ compensation benefits.

NOTE: To complete the process you must click the “Complete Incident” button on the final page and then confirm you are ready to complete the incident report by clicking “OK” to the pop up message. Your data will be lost if you close your browser without performing these final steps.
